A restaurant customer satisfaction survey template is a structured set of questions a guest answers after their visit or after collecting their order, to measure how satisfied they are with food quality, service, cleanliness, atmosphere and value for money. In this article you will find a complete, ready-to-copy template divided into clear sections, a short version for the table or a feedback device, and a practical way to analyse the answers and turn them into improvements. The template is designed for restaurants and cafés in Saudi Arabia, and you can easily adapt it to your type of restaurant.

What is a restaurant customer satisfaction survey and when do you need one?

A restaurant customer satisfaction survey asks guests directly about their experience instead of leaving management to guess what they think. You need one in many situations, including:

  • When you open a new branch and want to know how the first guests see it.
  • When you change the menu, a supplier or the head chef.
  • When you notice fewer visits or more complaints without an obvious reason.
  • On a regular basis, monthly or quarterly, to track the overall trend and compare branches.

Remember that the survey is one part of a wider system for measuring guest satisfaction, which we cover in detail in our guide to restaurant feedback from the owner’s point of view.

When and where should you present the survey?

The timing and place of the survey decide how long it can be. A guest waiting for the bill has about a minute; a guest who receives a link after ordering may give you two or three.

Timing Channel Suitable version
While waiting for the bill QR code on the table or receipt Full or medium version
On leaving Tablet or touch screen near the exit Short version (3 to 5 questions)
In quick-service restaurants Five-button device One overall satisfaction question
After a delivery arrives QR code on the bag or box Delivery version (accuracy, temperature, packaging)

Food quality survey: why it gets a whole section

Food quality is the main reason for a visit in most restaurants, but the question “Did you like the food?” on its own will not help you fix anything. A guest may love the taste but complain the dish was lukewarm, or be happy with the quality but find the portion small. That is why we split quality into taste, temperature, freshness, portion, presentation and variety: when one item drops, the kitchen knows where to step in. If you want a food quality survey on its own, for example after launching a new menu, take Sections 1 and 2 plus questions 25 and 29.

The short version for the table or a feedback device

If the full version is longer than your guest’s time allows, use this shorter version:

  1. Overall, how satisfied are you with your visit today? (1 to 5)
  2. How would you rate the quality of the food? (1 to 5)
  3. How would you rate the speed of service and the staff? (1 to 5)
  4. How likely are you to recommend us? (0 to 10)
  5. Is there anything you would like to tell us? (optional)

For quick-service restaurants, a single question on a five-button device is usually enough: “How satisfied are you with our service today?”, with a QR code for guests who want to add detail.

Delivery order version

  • Did your order arrive complete and correct? (Yes / No)
  • How hot was the food when it arrived? (1 to 5)
  • How would you rate the packaging? (1 to 5)
  • Your overall satisfaction with the order. (1 to 5)

Conditional questions: make the survey shorter and smarter

Not every guest needs to see every question. Conditional questions show the next question based on the previous answer, for example:

  • If a guest rates overall satisfaction 1 or 2, they see the question “What was the main reason?” with options (food, waiting, staff, cleanliness, price).
  • If they choose “Delivery” as the order type, the setting and cleanliness questions are hidden and the packaging and temperature questions appear.
  • If they answer “Yes” to first visit, they see the question “How did you hear about us?”.

This keeps the survey short for satisfied guests and detailed where you need the detail. RateHex supports this kind of conditional logic in its survey designer, with ready-made templates and the option to publish the same survey in Arabic and English through QR codes, tablets and five-button devices. You can explore the survey designer features to learn more.

How to analyse your restaurant survey results

1. Calculate the core metrics

  • Satisfaction rate: the number who chose 4 or 5 on the overall satisfaction question ÷ total respondents × 100.
  • Net Promoter Score: the percentage who chose 9 or 10 minus the percentage who chose 0 to 6 on the recommendation question.
  • Average for each item: to find the weakest food, service and setting items.

Illustrative example: 100 guests answered the recommendation question; 50 chose 9 or 10, 30 chose 7 or 8, and 20 chose 0 to 6. Net Promoter Score = 50% − 20% = 30. These figures are for illustration only and are not an industry benchmark.

2. Segment the results

Compare results by branch, by meal (lunch versus dinner) and by order type (dine-in versus delivery). The overall average can look healthy while one meal period has a clear problem.

3. Read the open comments

Group comments into themes (waiting, food temperature, a staff member’s conduct, price) and look for recurring words. One comment may be an isolated case; the same remark repeated points to a root cause.

4. Turn the result into action

Each month, choose just one or two items to improve, assign an owner and a deadline, then compare the results the following month. And do not forget the guests who asked to be contacted: replying to them quickly is part of the service experience itself.

Tips to raise survey response rates

  • Make the QR code clear and a sensible size, with a short line such as “Your opinion matters: one minute only”.
  • State the expected answering time at the start of the survey, and stick to it.
  • Ask staff to invite guests politely without standing beside them while they answer.
  • Offer the survey in both Arabic and English to serve all your guests.
  • If you offer an incentive, such as a voucher for the next visit, give it to everyone who responds regardless of their rating, so answers are not skewed.
  • Tell guests what has changed because of their feedback; it encourages them to take part again.

Common mistakes when designing a restaurant survey

  • Double-barrelled questions such as “How would you rate the food and service?”, so you cannot tell which one the guest means.
  • Leading questions such as “Didn’t you love our signature dishes?”.
  • An inconsistent scale, where 5 is best in one question and worst in another.
  • Collecting personal data without need or without clear consent. We recommend reviewing the requirements of Saudi Arabia’s personal data protection law from the official sources before collecting any contact details.
  • Changing the questions every month, so you lose the ability to compare over time.

Conclusion

A good restaurant customer satisfaction survey template is short from the guest’s point of view and detailed from management’s: a full version for guests with time, a short version at the table or exit, conditional questions that appear when needed, and regular analysis that turns numbers into decisions. Copy the template above, adapt it to your restaurant and start with one branch. For more templates in other sectors, see our customer satisfaction survey templates.
